They totally know it. Porn doesn't survive at all on youtube, but vaginal examinations and this photo are untouched, as well as other nude-but-not-porn stuff. They've gotten very good at defining and maintaining that thin line, so much so that it's gone on largely unnoticed. Except by people who try to upload porn to youtube, that is.

Well in most countries, except in the US and a few islamic countries, boobies and women bodies in general are perfectly fine and not considered "indecent" or "bad"...
